PROGRAM REQUIREMENTS
IN ORDER TO BE CONSIDERED FOR ADMISSION, AN APPLICANT MUST BE:
-Adult male (18+)
-Certified as homeless
-Clinically diagnosed with a substance use disorder
-Admitted to a licensed medical detox facility at the time of referral
ADMISSION STEPS
STEP 1: Submit The Online Intake Form
While at a medical detox facility, applicant may apply to Covenant Community, Inc by filling out the online intake form and attaching the following*:
-A mental health evaluation
-A tuberculosis screen (tb test)
-An RPR (Syphilis) test
-A recent physical examination
-Case worker's contact information
STEP TWO: Interview
CCI will invite the applicant for an interview, if appropriate
